Last week the clinic informed Buckley Air Force Base of the latest Cyberspace launch by the 460th Medical Group. The first of these initiatives is the Air Force Web Health Assessment tool.

This online tool will provide military members the ability to complete their Physical Health Assessment form online. Military members preparing for deployment or identified for their annual PHA can complete the first part of their PHA online.

One benefit is the member can complete the form on any computer with internet access, 24 hours a day.

The second benefit is that since the PHA form is online, time with the provider will be more efficient.

The PHA form can be found at https://www. defenserack.com/afwebha/.

The third benefit is the Air Force Web Health Assessment takes an average 16-18 minutes to complete.

And finally, the medical staff has access to the health assessment to review forms and determine appropriate care before the appointment.

The second initiative gives patients the ability to make their PHA appointment online. After reviewing the health assessment form, the medical staff will contact individuals via e-mail or phone informing them that they can now make their appointment. The member will then need to go to www.tricareonline.com, select 'appointments'; select 'physical health assessment' from the 'visit reason' drop down box; select the date and time of the appointment; and select the button 'I'll take it.'

The patient now has a PHA appointment, their form is complete and the remaining task, depending on the information on the questionnaire, is to see the provider.

Registration is required at the TRICARE online website if they haven't already done so.

The TRICARE online and Air Force Web Health Assessment are high profile Air Force Medical Service initiatives developed to improve services the Clinic provides patients. The combination initiative is undergoing testing and will be released soon.
